The Monsters Are Our Heroes: Tooth Fairy (aka Sugarplum Fairy)
We continue on our course of building monsters from "The Cabin In the Woods" with the Sugarplum Fairy. This monster was featured several times in the film, and was almost summoned to slay the protagonists. Had they allowed the ballerina music box to continue to play its song to the end, the Sugarplum Fairy would have been the monster to appear!
This monster build is based on "The Monsters Are Our Heroes" by Bloat Games

ATTACKS
Bite: +3 melee attack (1d4+3 piercing damage) and the target is grappled (escape DC 13).
Horrifying Appearance: All creatures Nearby the tooth fairy that can see her must make a DC 13 Wisdom saving throw or become frightened of the tooth fairy. A frightened target can repeat the saving throw at the end of their turn, removing the frightened condition on a successful save. Creatures that successfully save against a tooth fairy’s Horrific Appearance are immune to her Horrific Appearance for 24 hours.
ABILITIES
Faceless: Having no eyes, the tooth fairy is immune to being blinded. She perceives the area around her accurately up to Far distances using her auditory and olfactory senses. Areas of complete silence and overpowering scents can effectively blind and confuse a tooth fairy.
Fey Flesh: A tooth Fairy takes half damage from bludgeoning, piercing, and slashing weapons not made from cold iron.
Magic Resistance: A tooth fairy has Advantage on saving throws against spells and magical effects.
Sylvan Grace: A Tooth Fairy has Advantage on Dexterity (Stealth) and Charisma (Performance) checks. A tooth fairy adds her Dexterity score to melee attacks and damage
A tooth fairy appears to be a preteen girl dressed as a ballerina with the mouth of a lamprey filling the entirety of her face. Multiple rows of sharp teeth fill this mouth, leaving no room for a nose or eyes. She wears a golden ballerina dress and tutu, with matching shoes. The dress sparkles in the light.
It is rare when a tooth fairy is not seen dancing. This dancing is done to lure in potential prey, who think they are looking upon nothing more than a child at play. It is only when the victim is close enough, that the Tooth Fairy reveals their true visage and lunges towards their intended victim! They latch upon their targets with their mouths, chewing into their flesh and draining their blood. When their victim is slain, the fairy continues her dance over the mauled corpse.
